Healthy Moves for Aging Well

Healthy Moves for Aging Well is an in-home intervention designed to enhance the activity level of frail, high-risk sedentary seniors living at home.  The program has been proven to:


• Improve levels of physical activity in frail elders
• Improve fitness, strength, confidence, and independence
• Reduce depression, pain, number of falls, and fear of falling


Who should consider taking Healthy Moves for Aging Well?

Older adults who are currently enrolled in a care management program
