2019-2020 Mini-Grant Announced for Wisconsin Community-Based Organizations

For Stepping On and Chronic Disease Self-Management Education Programs


The Wisconsin Institute for Healthy Aging is pleased to announce the availability of mini-grants for the period January 1, 2019-July 31, 2020 to improve the health of your community by developing or expanding evidence-based health promotion programs in community-based organizations that make a long-term commitment to on-going implementation and delivery of the selected programs to become strong, committed and effective program providers. 

 

Eligible programs are:

Stepping On Falls Prevention

Living Well with Chronic Conditions

Tomando Control de su Salud

Healthy Living with Diabetes

Vivir Saludable con Diabetes

Healthy Living with Chronic Pain
